Applicant must submit proof of Confederate Military Record, line of descent from Confederate ancestor, a letter of endorsement from sponsoring Chapter, a letter of recommendation, complete financial report, official transcripts, an essay on the significance of a southern heritage, and a wallet-size photo. Original and four copies of the application forms and five copies of all documents, certificates, and letters must be submitted to the Division Director of Education.
