Applicant must obtain application from the medical school authorities, complete it and return it to them for forwarding to the Foundation. Selection is based upon financial need, scholastic record and standing, demonstrated interest in arts and letters or other cultural pursuits outside the field of medicine, intention to specialize in neurology, psychiatry, or general practice, evidence of good moral character, and attendance at an accredited medical school located east of or contiguous to the Mississippi River.

A loan is available to those students incurring sudden need while enrolled in a nurse anesthesia program. Applicant must demonstrate interest and involvement in the communication of science. Applicant must be nominated by the dean of an AMA-approved medical school. The following materials (original and one copy) should be submitted: letter of nomination from the Office of the Dean (addressed to J. Applicant must be a lineal descendant of worthy Confederates or collateral descendant who is a member of the Children of the Confederacy or the United Daughters of the Confederacy. Applicant must have at least a 3.0 GPA, be enrolled in an accredited college or university, and carry a minimum of 12 hours each semester. Applicant must be prepared to pass entrance examination of prospective college or university. Applicant must submit proof of Confederate Military Record, line of descent from Confederate ancestor, a letter of endorsement from sponsoring Chapter, a letter of recommendation, complete financial report, official transcripts, an essay on the significance of a southern heritage, and a wallet-size photo. Original and four copies of the application forms and five copies of all documents, certificates, and letters must be submitted to the Division Director of Education. Award is to encourage specialized training early in a physician's career and to prepare candidates for careers in academic medicine. Training will take place in a Cystic Fibrosis Foundation care center and must encompass diagnostic and therapeutic procedures, comprehensive care, and cystic fibrosis-related research.
