Must reside in or around the following cities: Buffalo, NY, Boulder, CO, Chicago, IL, Columbus, OH, Denver, CO, Kansas City, KS, Knoxville, TN, Missoula, MT, Omaha, NE, Pittsburgh, PA, Rapid City, SD, Salt Lake City, UT, or Spokane, WA. Essay, physician verification form, and letter of recommendation required.
